Infertility is a growing problem worldwide and IVF has been a boon in helping many couples and individuals starting a family. IVF or In – Vitro Fertilization is a type of Assisted Reproductive Technology (ART) that involves fertilization of an egg with a sperm outside the body in a laboratory setting. However, IVF has limited success rate and the success of IVF largely depends on various factors including the protocols used. Here we will explore the different protocols commonly employed in IVF and their unique characteristics.
TYPES OF IVF PROTOCOLS
In an IVF cycle, protocol refers to the specific plan for ovarian stimulation. Essentially, it outlines the medications you’ll take and when you’ll take them to stimulate the ovaries to produce multiple eggs.
Also known as agonist protocol, is the traditional IVF protocol that’s been in use since the early days of IVF. It involves suppressing the natural menstrual cycle through a medication called Lupron/ Leuprolide acetate. This suppression prevents the release of eggs prematurely. After this, ovaries are stimulated by administering Follicle stimulating Hormone (FSH) injections. Later the eggs are retrieved through a minor surgical procedure and fertilized with sperm in a lab and the resulting embryos are then transferred into the uterus. The entire process typically takes 4 – 6 weeks, hence the name.
This is a more time – efficient option. It involves directly stimulating the ovaries with FSH injections without the initial suppression of natural menstrual cycle. After a few days, a medication known as GnRH antagonist is introduced to prevent premature ovulation. It typically takes about 2 weeks to complete and involves fewer days of hormone injections compared to long protocol.
This type of protocol doesn’t involve any ovarian stimulation or use of fertility drugs. This mimics the natural menstrual cycle and only the single dominant follicle that develops during the cycle is selected for egg retrieval. However, since only one egg is retrieved, the success rate may be lower compared to the conventional IVF protocols.
Also known as Mini – IVF or Gentle IVF. This involves using lower doses of fertility drugs to stimulate the ovaries. The aim is to produce a smaller number of high-quality eggs while minimizing the risk of Ovarian Hyperstimulation Syndrome (OHSS) and reducing the cost of medication.

The choice of protocol for IVF depends on various factors, including the individuals age, ovarian reserve and previous response to fertility treatments.
